Warm, calm, and familiar—not institutional
We don’t just provide care—we create a place where residents feel safe, valued, and connected, and where families feel confident and at peace. How do you create care plans for residents?
We develop each resident’s care plan through a structured and personalized process to ensure their unique needs, preferences, and goals are fully understood and supported.
Before admission, we complete a comprehensive assessment that looks at the individual’s medical needs, daily living abilities, preferences, and personal history. Once the resident moves in, we create a preliminary service plan immediately to ensure safe and consistent care from day one.
Within the first 30 days, this is expanded into a detailed Negotiated Care Plan, which is developed collaboratively with the resident (and family or representative, if applicable). This plan outlines:
The specific care and services provided
Daily routines and preferences
Medication management
Physical, emotional, and social needs
We believe care should always be person-centered, meaning we adapt our approach to fit the resident—not the other way around. Care plans are living documents that are regularly reviewed and updated to reflect any changes in health or preferences. How do you ensure resident safety 24/7?
